50th Street Care, located at 1495 50th Street, San Diego, offers a warm, family-style home for adults with developmental disabilities. The two-level residence features six private bedrooms and spacious shared areas for dining and relaxation. Future upgrades will enhance accessibility and comfort. Conveniently situated near parks, shops, and transit, it encourages community connection and engagement.

Daily Living Support

A Safe, Comfortable Place to Call Home
Our two-story home, located at 1495 50th Street, San Diego, CA 92102, is fully equipped and designed for comfort, accessibility, and community living.
Home Features:
- Six private bedrooms for individual comfort and privacy
- Spacious common areas: living room, dining room, and kitchen
- Staff room, office, and multiple bathrooms
- Large fenced backyard and inviting outdoor spaces
- Located near shopping, parks, and public transit
We encourage residents to decorate their rooms and make the space their own — creating an environment that feels like family.
- Admissions
Join the 50th Street Care Family
We currently accept up to six residents who meet the following criteria:
- Ages 18–59
- Ambulatory (able to move independently)
- Diagnosed with mild to moderate developmental disabilities (e.g., intellectual disability, cerebral palsy, epilepsy, autism)
- Eligible for San Diego Regional Center (SDRC) services
- May have mild to moderate self-care deficits or behavioral challenges
Admission Process
Referral & Intake:
We coordinate directly with SDRC and families to begin the intake process.
Pre-Placement Visit:
Prospective residents are encouraged to visit the home, meet staff and residents, and experience our environment.
Assessment & Transition:
Our team develops an individualized plan and provides full support during the transition into the home.
Ongoing Support:
Residents receive quarterly and annual reviews to ensure their goals are met and their needs evolve with care.
We partner with families, caregivers, and case managers to ensure smooth transitions and consistent communication.

We follow the principles of Normalization and the Home and Community-Based Services (HCBS) Final Rule, ensuring residents:
